Once registered, you will then be sent an email link to set up a patient portal. This will establish a direct HIPAA compliant connection to communicate via EMR (electronic medical record). Screeners will be sent to you via this portal along with a clinical history form. This will need to be completed prior to your first appointment.

When you arrive for your first appointment, please have the following information with you:

• Photo ID

• Insurance Card

• Active Credit Card for Billing
